Abstract

Introduction: The aim of this study is to evaluate the results of the first 500 open heart operations performed in the Cardiovascular Surgery Clinic of Kocaeli Derince Training and Research Hospital. Patients and Methods: A total of 500 patients (360 male, 140 female with a mean age of 61.23 years) were operated as open cardiac surgery between April 2010 and October 2014. 414 cases were operated for coronary artery disease, 21 were for aortic aneurysm and dissection, 18 were for aortic valve disease, 14 were for mitral valve disease, 10 were for atrial septal defect, 3 were for myxoma, 1 was for aortic fistula, 1 was for mediastinal mass; the others were for combined cardiac valve and coronary arterial disease, combined aortic aneurysm and coronary arterial disease and cardiac stabbing from an external center. Results: The in-hospital mortality was 18 patients (3.6%). Sixteen of these were coronary artery surgery, one was mediastinal mass excision and one was cardiac stabbing. Atrial fibrillation occurred in 137 parients who were followed up in the intensive care unit and inpatient room. Wound infection occurred in 11 patients, mediastinit occurred in 3, low cardiac output occurred in 33 patients, kidney failure occurred in 5, respiratory insufficiency occurred in 12, neurologic disfunction occurred in 7 and atrioventricular block occurred in 2 patients. Intraaortic balloon pump were placed in 23 of the patients with low cardiac output. Two patients were implanted permanent pacemakers. Fifteen patients were taken to reoperation for bleeding and sternal dehiscence. Conclusion: Mortality and morbidity rates are in an acceptable level for our hospital as a center which started open heart surgery with Kartal Kosuyolu Yuksek Ihtisas Training and Research Hospital as coordinator. Although there are seven centers performing open heart surgery in Kocaeli, our center has risen to the first place when evaluated for number of operations and rates of survival in the recent years.
